NOTE: Your division will be assigned the day of event depending on how many people register in your age & rank group. Our computer system will add divisions accordingly. Onsite day-of event sign-in and registration starts promptly at 8:30 a.m.

WAIVER AND RELEASE OF LIABILITY

Flores Bros. Kenpo Karate Studios, Inc. Invitational Karate Championships In consideration of being permitted to participate in the activities conducted by Flores Bros. Kenpo Karate Studios, Inc., I agree as follows on behalf of myself, my personal representatives, heirs, beneficiaries and assigns.

I ACKNOWLEDGE AND AGREE that my participation with Flores Bros. Kenpo Karate Studios, Inc. is for the purpose of practicing martial arts; the practice of martial arts is physically strenuous, dynamic, and challenging practice of self defense which includes, but is not limited to, punching, kicking, striking and grappling, both individually and with partners. I further understand that such activity includes inherent dangers including serious bodily injury, permanent disability, paralysis or death which may be caused from my actions or inactions, the actions or inactions of others or the conditions of the facility in which the activity is conducted; I understand there may be other risks or other social or economic losses either not known to me or readily foreseeable at this time; and I FULLY ACCEPT ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES incurred as a result of my participation in the activity. I am responsible for monitoring my own activity level and maintaining awareness of my own physical limitations. I further agree that I will immediately discontinue participation in any activity that I perceive to be unsafe for any reason.

I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE AND AGREE TO HOLD HARMLESS Flores Bros. Kenpo Karate Studios, Inc., its administrators, members, directors, agents, officers, volunteers, employees and other participants, owners and lessors of the premises on which the activity takes place (each considered a “Releasee”) from all liability, claims, demands, losses or damages on account caused from participation. I further state that myself/child is in proper physical health and condition to participate in this karate tournament. I am also aware that this program reserves the right to exclude admittance or competition to those who engage in disruptive behaviour or are deemed by the Judge(s) to be counterproductive to the ideas of Flores Bros. Kenpo Karate Studios, Inc.. I further agree and consent that any pictures or video taken or captured of me as a martial arts competitor, student or observer can be used for publicity or promotion in any fashion including publication or broadcasting on television, internet or other mass media by Flores Bros. Kenpo Karate Studios, Inc. I also waive compensation thereto.
